The following information is intended to assist prospective grantees in preparing requests for funding. We encourage you to provide information in concise statements. Lengthy explanations are not necessary, but the more specific you can be, the better.

It is important to identify grant progress measures, success and outcomes> at the beginning of the grant and to collect data through out the grant period. We encourage grantees to set measurable goals and to specify what type of data you will collect to measure results and how often that data will be collected.
